Nortriptyline is a second-generation tricyclic antidepressant marketed as the hydrochloride salt under the trade names Aventyl and Pamelor. It is used in the treatment of major depression and childhood nocturnal enuresis. Its off-label uses include treatment of chronic fatigue syndrome, chronic pain and migraine, and labile affect in some neurological disorders. It may also be used to help people quit smoking and to treat attention deficit hyperactivity disorder. Nortriptyline comes as a capsule and an oral liquid to take by mouth.

Every patient is different. You may notice some initial benefit within 2 weeks, however it may take up to 2 months for a full effect. Your doctor may need to increase the dose to get the maximum effect. If you do not feel any improvement in your pain after 2 months, do not suddenly stop taking the tablets but speak to your doctor.
Usual adult dose - 25 mg three or four times daily; dosage should begin at a low level and be increased as required. As an alternative regimen, the total daily dosage may be given once a day. When doses above 100 mg daily are administered, plasma levels of nortriptyline should be monitored and maintained in the optimum range of 50 to 150 ng/mL. Doses above 150 mg/day are not recommended.

In depression it may take between 1 to 4 weeks before amitriptyline starts to work and you begin to feel better. Usually recommend continuing antidepressants for at least six months after recovery. With nerve pain amitriptyline may start to help in a few days for some people, while others don't see much difference for 1-2 weeks.
Usual adult dose for depression: 75 mg orally per day in divided doses; this may be increased to a total of 150 mg per day if needed.

This med is amazing. I've had peripheral Neuropathy for a few years in my feet. I've tried Gabapentin, it didn't work. I tried Nortriptyline, I've been on it for 3-4 weeks...it's amazing... NO pins/needles feeling, no burning sensation! I still have some pressure in my feet, but things are so different now! I'm going to give it more time, but I think I found my holy grail! I wasn't prescribed 10mg of amitriptyline for migraines and to help me sleep. This drug is great for helping you sleep. I was drowsy within 10 mins of taking it at night before bed. However, as a migraine prophylactic it did not help. I had migraines occurring frequently and often worse than they were prior to starting this medication. I also gained 18 lbs in 4 months. My breasts became very swollen and tender and I ended up extremely depressed. My dosage was the lowest you can get. I cannot even imagine taking more than I did and the side effects it would have had. I don't recommend this to anyone.
I have had insomia for months. Dr started me on Elavil 10mg for sleep, within a half hour I'm a sleep. I have not slept this good in a long time. I am not a zombie either in the mornings. I feel very good when I awake. Best thing.
If I take amitriptyline in the morning, I am so tired that functioning and performing even small tasks is very difficult. But if I take it too late at night, I have really bad nightmares. So find a time that works best for you.
